<p>We are deeply saddened to hear of this tragedy and our hearts and thoughts go out to the victims’ loved ones. We wish a full and fast recovery to the accident’s two survivors.</p>
<p><strong>Increasing Awareness to Maximize Safety</strong></p>
<p>This air ambulance fatality is a devastating reminder to take a closer look at safety in air ambulance transport. We cannot change the outcome of  this tragedy, but we can arm you with information that will help you to make the best and safest decision in choosing an air ambulance provider.</p>
<p>The fact is, arranging a medical flight is not something most people do every day; it’s probably not something most people will do in their lifetimes. But it is what <em>we</em> do every day, and we want to ensure that you understand the requirements for a safe, successful medical flight.</p>
<p>Here are some things you should expect in an air ambulance provider:</p>
<p><strong>Well-maintained, medically-dedicated aircraft </strong></p>
<p>Some operators own their aircraft; some have exclusive leases with reputable providers; and some “companies” are actually brokers who broker out flights to the cheapest provider so that they can make the most profit. These distinctions can affect safety and quality of service.</p>
<p>When providers operate their own aircraft or have exclusive operating agreements, they know exactly which aircraft patients are flying on, including condition, maintenance history and safety features. When brokers are choosing operators based on price, there is more likelihood that corners are being cut somewhere along the line. (In this industry, be aware that the best price may not always be the safest decision.)</p>
<p>Ask the following questions:</p>
<ul>
<li><em>Do you own aircraft yourself or have exclusive use arrangements with specific providers?</em></li>
<li><em>What type of aircraft will my loved one be transported on?<br />
</em></li>
<li><em>What safety features does the aircraft offer? </em></li>
</ul>
<p>Jet Rescue Air Ambulance has an operation agreement in place for the operation of its medically-dedicated Learjets 35s.  We get involved on who’s maintaining the jets, who’s flying them and what condition they’re in. The jets are outfitted with advanced avionic equipment including GPS navigation,Color weather radar, and the latest in collision and terrain avoidance systems (EGPWS, TCAS/TCAS-II, TAWS, etc.).</p>
<p>Each fully customized medical jet is equipped with a self-contained Lifeport  stretcher,critical care equipment, medications, and the latest technology in medical devices and monitoring capability; they are flying intensive care units .</p>

<p>The flight crew members  that fly on our Learjets have thousands of hours in the air including significant experience in Learjets. Two pilots are in the cockpit on every flight. All pilots have completed simulator training in the aircraft, and have fulfilled or exceeded all FAA requirements.</p>
<p><strong>Experienced, compassionate medical crews prepared for any situation</strong></p>
<p>It is not an uncommon air ambulance industry practice for medical flight crews to have full-time jobs <em>and</em> moonlight as flight nurses/paramedics. <p>Every member of the Jet Rescue medical crew is a full-time employee with years of experience in emergency medicine. Our  medical crews undergo an extensive orientation and training process including flight Physiology and aviation safety prior to flying with patients. Additionally, all medical personnel participate in 100 hours of didactic and clinical education each year, including training on the Laerdal Sim man.</p>
<p>Both a critical care flight nurse and paramedic accompany the patient throughout the entire transfer. Together, these highly-competent, compassionate teams can handle virtually any incident that might occur during a transport. Every year thousands of senior retired citizens travel south to escape the cold winter weather in the USA and Canada. Statistics sadly enough show that senior citizens are more likely to get sick or injured while travelling.</p>
<p>Let´s take a look at common causes for (elderly) people needing our air ambulance service.</p>
<p>Respiratory Failure</p>
<p>Respiratory failure is a syndrome in which the respiratory system fails in one or both of its gas exchange functions. Examples of respiratory failure are cardiogenic or noncardiogenic pulmonary edema, pneumonia, and pulmonary hemorrhage.</p>
<p>It frequently occurs that we transport a patient diagnosed with pneumonia. Pneumonia is an inflammatory condition of the lung. It is considered a common illness which can occur in all age groups. It is however a leading cause of death amongst the elderly and people who are chronically and terminally ill.</p>
<p>Heart Problems</p>
<p>Heart problems such as heart failure are a common reason for an air ambulance transport. Heart failure is generally defined as inability of the heart to supply sufficient blood flow to meet the body&#8217;s needs. It has various diagnostic criteria, and the term heart failure is often incorrectly used to describe other cardiac-related illnesses, such as heart attack or cardiac arrest.</p>
<p>People that have suffered a heart attack while on vacation need medical evacuation. Most heart attacks happen when a clot in the coronary artery blocks the supply of blood and oxygen to the heart. Often this leads to an irregular heartbeat that causes a severe decrease in the pumping function of the heart. A blockage that is not treated within a few hours causes the affected heart muscle to die.</p>
<p>Head Injury</p>
<p>A head injury is any trauma that leads to injury of the scalp, skull, or brain. The injuries can range from a minor bump on the skull to serious brain injury. <p>Brain Hemorrhage is a condition caused by a sudden stroke to a person after blood leaks out from the blood vessels in the brain. This situation occurs due to the break in the wall of blood vessel, the blood spills out of the blood vessel and enters the area where vital tissues and cells of brain reside, killing those tissues and cells; this causes the patient to become seriously ill and needs immediate medication and/or treatment.</p>
<p>Cranial Base Fracture</p>
<p>A skull fracture is a break in one or more of the bones in the skull usually occurring as a result of blunt force trauma. If the force of the impact is excessive the bone may fracture at or near the site of the impact. The force of direct impact may cause damage to the underlying physical structures contained within the skull such as the membranes, blood vessels, and brain, even in the absence of a fracture.</p>

			<content:encoded><![CDATA[<p>Mexico City &#8211; <a title="www.jet-rescue.com" href="http://www.jet-rescue.com" target="_self">Jet Rescue Air Ambulance</a> is proud to announce the opening of a new business unit based in Mexico City. The &#8220;<a title="www.criticalair.com.mx" href="http://www.criticalair.com.mx" target="_blank">Critical Air</a>&#8221; business unit offers emergency evacuation by means of its A-119 Koala air ambulance helicopter.</p>
<p>Quick response teams are available 24 hours a day, 365 days a year, in all weather conditions to ensure rapid and reliable medical retrieval in times of emergency. The helicopter, an A-119 Koala, is specially equipped to transport patients and medical crew. The helicopter can be deployed to even the most remote locations.</p>
<p>The A-119 Koala Helicopter is unique amongst single turbine powered helicopters, with a cabin volume that is approx 30% greater than other helicopters in its class. It has a cruise speed of 270 km/h and a maximum range of 1000 km.</p>
<p>The specially equipped air ambulance helicopter can accommodate up to two stretchers together with the medical crew. The internal layout includes provision for the installation of all necessary medical equipment to treat patients during transport. Two standard rails are positioned both in the cabin rear bulkhead and on upper side panels that include outlets for oxygen, emergency air and power.</p>

Not many people know that Jet Rescue pilots actually go through an extensive volcanic cloud training program. They are trained how to deal with volcanic clouds when flying. Below you will find information on how volcanic cloud training is addressed.</p>
<p>The <a title="www.jet-rescue.com" href="http://www.jet-rescue.com" target="_self">Jet Rescue Air Ambulance</a> pilots are tested on the correct implementation of the procedures underneath when a volcanic cloud is recognized by the pilot and in case the cloud could not be avoided. Each operator&#8217;s flight operation manual includes more specific directions.</p>
<ol>
<li>Reduce thrust to idle immediately. By reducing thrust, engines may suffer less buildup of molten debris on turbine blades and hot-section components. Idle thrust allows engines to continue producing electrical power, bleed air for pressurization, and hydraulic power for airplane control.</li>
<li>Turn the auto-throttles off. This prevents the engines from increasing thrust above idle. Ash debris in the engine can result in reduced surge margins, and limiting the number of thrust adjustments improves the chances of engine recovery.</li>
<li>Exit the ash cloud as quickly as possible. A 180-deg turn out of the ash cloud using a descending turn is the quickest exit strategy. Many ash clouds extend for hundreds of miles, so assuming that the encounter will end shortly can be false. Climbing out of the ash could result in increased engine debris buildup as the result of increased temperatures. The increased engine build-up can cause total loss of thrust.</li>
<li>Turn on engine and wing anti-ice devices and all air-conditioning packs. These actions improve the engine stall margins by increasing the flow of bleed air.</li>
<li>If possible, start the auxiliary power unit (APU). The APU can power systems in the event of a multiple-engine power loss. It can also be used to restart engines through the use of APU bleed air.</li>
<li>If volcanic dust fills the flight deck, the crew may need to use oxygen. Use flight deck oxygen at the 100 percent setting. Manual deployment of the passenger oxygen system is not required because it will deploy automatically if the cabin altitude exceeds 14,000 ft.</li>
<li>Turn on the continuous ignition. Confirm that auto-start is on, if available. In the event that the engines flame out or stall, use appropriate procedures to restart the engines. During restart, the engines may take longer than normal to reach idle thrust due to the combined effects of high altitude and volcanic ash ingestion. If an engine fails to start, try restarting it again immediately. Flight crews should remember that the airplane may be out of the air-start envelope if the encounter occurs during cruise.</li>
<li>Monitor engine exhaust gas temperature (EGT). Because of potential engine debris buildup, the EGT can climb excessively. The flight crew should prevent EGT exceedances. Shut down the engine and restart it if the EGT is approaching limits similar to a hung start.</li>
<li>Fly the airplane by monitoring airspeed and pitch attitude. If necessary, follow the procedure for flight with unreliable airspeed.</li>

<p><strong>Necessity..<br />

<p>Escorts may be necessary when a patient requires investigation or treatment at a hospital located far away from their current location. The escort may involve accompanying the patient to and / or from the receiving hospital. This may include a short term or long term stay for the patient.</p>
<p>Whatever the circumstances, escorting staff and managers need to be clear about what is expected of them, and their accountability to the patient and their employer. This policy is designed to offer advice and guidance, but cannot cover all eventualities.</p>
<p><strong>General Principles </strong></p>

<p><strong>Assessment and communication of care needs before transfer </strong></p>
<p>Preparation and communication are important. Whether the transfer be planned or unplanned, there should be contact between Jet Rescue Air Ambulance and the receiving hospital before the transfer, to ensure that the patient is expected, and the patient’s care needs have been explained to the receiving hospital.</p>
<p>It is recommended that a referral to the hospital is made in writing, preferably in advance of the journey. In an emergency a phone call may be sufficient, however in all cases a copy of a referral letter should accompany the patient. Medical and nursing notes can be brought with the patient by the escort staff, but should not be left at the receiving hospital as they are the property of Jet Rescue Air Ambulance. If necessary, photocopies of relevant medical and nursing notes should be arranged before the transfer. A transfer form, containing relevant medical and nursing information should also be completed, and can be left with the patient, as long as a copy remains in the Jet Rescue Air Ambulance medical / nursing notes.</p>
<p>An assessment should be made of whether the patient requires an escort, and if so, what level of staff is required. Some patients may be fit enough to travel on their own, using their own transport, or be escorted by a relative or volunteer. At the other extreme, a critical ill patient in ITU may require a full medical team. This decision should be made by the Director of Operations, seeking advice from the medical staff where necessary, and taking into account risk factors.</p>
<p><strong>Duty of care </strong></p>
<p>Jet Rescue Air Ambulance has a duty of care to ensure that its patients are safe. This duty of care continues while the patient is under the care of Jet Rescue Air Ambulance, which would include transit to or from a hospital.</p>
<p>The patient remains in the care of Jet Rescue Air Ambulance, up until the point that handover takes place between Jet Rescue Air Ambulance staff, and the receiving hospital’s staff. At the conclusion of handover, the receiving hospital should assume full responsibility for all care needs of the patient. On completion of the treatment, it should be the responsibility of the receiving hospital to ensure that the patient is reassessed and re-stabilized before handover to Jet Rescue Air Ambulance.</p>
<p>Should the handover be delayed beyond what it reasonably expected, the member of staff escorting the patient should draw the matter to the receiving hospital’s attention, seeking management support where necessary. Should this not resolve the issue, the escort should contact the Jet Rescue Operations Director (or out of hours, the site manager) at Jet Rescue Air Ambulance to seek their advice.</p>
<p><strong>Employment issues </strong></p>
<p>The role of the escorting staff is to:</p>
<p>1) Ensure the patient arrives at the receiving hospital safely</p>
<p>2) Hand over care to the receiving hospital’s staff</p>
<p>3) Accept hand over from the receiving hospital’s staff and to return the patient safely.</p>
<p>As a general rule, an employee of Jet Rescue Air Ambulance would require an employment contract in order to practice on hospital’s premises. In the case of staff visiting a hospital for practice experience or teaching, for example, an honorary contract may suffice, however this is unusual in the case of escorted patients.</p>
<p>As part of their duty of care, Jet Rescue Air Ambulance staff must provide care up and to the point of handover of the patient to the receiving hospital, but <span style="text-decoration:underline;">once handover has been effected</span>, the receiving hospital can not provide vicarious liability to an escort nurse, unless an honorary contract is in place. Neither can Jet Rescue Air Ambulance be held liable for care given by its own staff on a hospital’s premises, once the receiving hospital has accepted a duty of care for that patient following handover. Staff is therefore strongly advised not to carry out clinical duties once the care of the patient has been handed over, even for a short period of time. They must however ensure the patient is safe before and during the period of handover.</p>

<p>Many people ask themselves how an air ambulance copes with difficult weather circumstances such as we are seeing this winter. Fast moving fronts, ice fog, strong winds, snow and flat light conditions are part of the winter period. These conditions require careful planning in order to minimize their effects. For these kinds of conditions special winter operating procedures are in place. Let us have a look at both take-off and landing as this is regarded by most people as the most delicate part of flying and aircraft</p>
<p><em> </em></p>
<p><em>Take-off</em></p>
<p>Before an air ambulance aircraft takes off, the second officer checks if there is ice on the wings and reports his findings to the air ambulance captain. For take-off it is necessary to have the wings clear of ice or anything that will adhere to the wing and cause a reduction in lift or alter the aerodynamics of the wing. In case ice was found on the wings the captain calls in the ground crew for de-icing the air ambulance.</p>
<p>After a call from the air ambulance captain the ground crew heads to the air ambulance and de-ices the airplane with heated de-icing fluid which consists of Ethylene or Propylene Glycol, diluted with water. This special fluid is sprayed, under high pressure, onto the air ambulance to melt the frost, ice and snow. Once the airplane is de-iced, the air ambulance is ready for take-off. The de-icing takes place about 15 minutes before take-off.</p>
<p><em>Landing</em></p>
<p>In winter conditions the runway surface can be very treacherous. Before landing advanced information about the current conditions of the runway is obtained by our air ambulance captain.</p>
<p>When landing is in process, visibility is of the upmost importance. The air ambulance co-pilots` job is to monitor the aircraft instruments. He is in charge of making sure that the control navigation system is working correctly. The air ambulance captain is in charge of monitoring the instruments until the aircraft gets near visibility minimums. If the runway lights are in sight, and the aircraft is due to snow or rain at or above visibility minimums, the air ambulance captain will disconnect the autopilot and land the air ambulance manually.</p>
<p>Winter conditions such as ice fog and snow can only make flying dangerous if winter operating procedures are not in place.
